Job responsibilities include client intake, establishing services, creating care plans, and following client care until completion of services for 100 clients. This job involves extensive paperwork to meet state guidelines and timely completion of tasks. The care manager must also handle changes in client's needs which require daily monitoring and direct contact with contracted vendors that provide for their care.
