I was the immediate supervisor for a team of 12 direct service staff serving clients in the central San Fernando Valley for the Community Integration Program, better known as “CIP”. I provided direct management of staff, quality assurance and case management services for 32 clients and serve as the foundation's liaison with community partners within my geographic area. Under the direction of the CIP Program Director, I collaborate with clients, families and other CIP team members to develop opportunities for clients to participate in adult education, community service, paid work and recreational/cultural activities of their choosing. I conduct all of my client's person centered planning process, which identifies desired goals, objectives and action plans.Client's Life Planning/Discovery As Area Manager, I assist with the development of client referrals, monitoring and maintaining a client referral list. I am also in charge of ensuring effective transitioning of clients into and out of the CIP program by actively participating in Life Planning/Futures Planning meetings, the preparation of individual transition plans and providing all direct support staff with relevant background information of new CIP clients. When clients leave Tierra services from my area in CIP, I am also responsible for completing the client's Exit Summaries and conduction Follow Up Surveys at 3 months and at one year after the client has departed. In my role I also conduct, monitor and maintain Individual Futures Plans as well as the initial 90-day discovery process. Write semi-annual and annual review summaries and distribute to all necessary parties. Monitor Individual Service Plans for each client, and assist job trainers with the implementation of plans. Arranging for the development of Individualized Behavior Support plans.
