APN-provides direct patient care and advocacy for survivors of childhood cancer. Sole practitioner in CBCC Pediatric Survivorship Clinic under the supervision of the child's primary oncologists.
- Duties: physical exams, ordering and interpretation of lab tests, x-rays, scans, echo/ekg and bone density studies and all other tests and studies for monitoring of disease relapse and surveillance of late effects of cancer treatments. Referrals to specialty clinics, social service and dietary consultation, play therapy and child life interventional services, psychological and psychiatric services. Referrals to adult primary care and specialty providers and adult psychiatric services.
- Participates,prepares documentation and educates patients and families on the Passport For Care program.
-Follows the off treatment protocols to provide ongoing monitoring of clinical laboratory and disease evaluations after completion of therapy.
-Survivorship Program development: oversees the responsibilities of the clinic team members which consist of a clinical dietician, clinical social worker, medical assistants and office coordinator.
-Adolescent and Young Adult Survivorship Program (AYAC)-works in collaboration with the AYAC nurse navigator to ensure optimal transition of care from pediatric to adult cancer survivorship.
